The British Columbia Coroner released a final cause of death report on Wednesday, indicating that ‘Glee’ actor Cory Monteith died “accidentally” of combined heroin and alcohol toxicity.

According to the information made public on Oct. 2, 2013, Corey Monteith’s death was attributed to injecting “intravenous heroin combined with the ingestion of alcohol. There was no evidence of fatal injury, illness or disease.”

The report detailed the fact that the actor was found unresponsive on the floor on July 13 by a Pacific Rim Hotel staff employee in Vancouver. Investigators found drug paraphernalia, including “a spoon with drug residue and a used hypodermic needle, along with two empty champagne bottles and glasses.”

Toxicology findings indicate Monteith had an alcohol level of 0.13 percent, a “moderate” level of intoxication. Other substances discovered during testing: morphine, codeine and 6-monoacetyle morphine.

The coroner’s report included hotel video captured of Monteith returning alone to his room at 2:16 a.m. on July 13, the last time he was seen alive.

At the time of his death, the 31-year-old Canadian actor was living in Los Angeles, where ‘Glee’ is filmed. Besides his parents, he is survived by his girlfriend of a year and a half, actress Lea Michele.
