This son is in the autism spectrum somewhere. I have a shirt-tail cousin with Asperger's (now an adult), and he could be a holy terror at that age. I wonder why--if this story is even true--he has picked out his grandma for this kind of "attention." Perhaps, he has observed her and his mother in heated discussions and has gotten the idea that she's bad news, so he's protecting his mom. But it seems to me that Nadya is doing quite well at raising her kids--and remember that she also has nannies in the house, so it's hard for me to even believe this story, as there would have been several adults around to have calmed him down. Seems to me as if this story is either totally-untrue or else is only partially-true, but it's just more sordid gossip to try to bring Nadya down.

About her back injury...it restricted her from doing all of the work she did before, but she worked at the same place in other capacities long after this happened.
Unless you're going to describe any baby/child photographed frequently due to being unique in some way (a multiple, sibling of multiples, child of celebrities, etc.) as being born to work, this definition doesn't fit Nadya's kids.
Nadya's mom doesn't see the problem with the photographers, saying that the babies are too young to even be aware of them and that the older kids enjoy the attention.
This seems like an honest way to raise money.
Around here, owners of small businesses have been known to include their kids and/or grandkids in commercials from the time they're small.
We have this one business that has sold pools and other outdoor equipment for years.
Their daughter (when I first noticed the commercials) was a teenager at the time. Likely, she was probably a child when she first started being included.
She got married a few years ago, and she and her husband are now expecting their first child, and she's doing the commercials obviously pregnant.
My guess is that she will probably end up doing commercials holding her baby in time.
Anyway, imo, no child is being exploited unless he/she is being harmed.
