It's unsurprising but official: the Cory Monteith autopsy results are in and this actor died from a mixture of heroin and alcohol.
Sources close to the Glee star and the Glee set, meanwhile, say cast members and producers saw the actor headed down a dangerous path.
They knew his drug habit had returned, they knew specifically that heroin was the issue, and they pushed Monteith to enter rehab this spring.
Those close to the star sat him down for an "open conversation," an insider tells TMZ, and it "wasn't met with resistance."
"He was always so grateful and aware of how fortunate he was," the Glee source says. "The decision was to best help our friend. The needs of the show fell second to his well-being."
Monteith listened at the time and checked in to a substance abuse facility on March 30.
He came out a few weeks later, smiling and supposedly on his way to a clean bill of mental and physical health.
But at least one significant setback took place this weekend and the actor is no longer with us. He was 31 years old at the time of his death.
