Hugo Avalos-Chanon, a 41-year old employee for Hugo Avalos-Chanon in Oregon, was killed Friday when he tumbled into a running blender at a meat-processing plant.
The Clackamas County Sheriff's Office confirmed the passing, saying paramedics and law enforcement officials were called just prior to midnight and found Avalos-Chanon's stuck in the machine, as a worker could not press the emergency stop button in time to save him.
Investigators have ruled out foul play and the state medical examiner has listed the cause of death as "blunt-force injuries and chopping wounds."
DCS Sanitation Management's operations were inspected in 2001, 2002 and 2004.
No violations were found, but Occupational Safety and Health Administration officials will examine all evidence in this case, interview witnesses and review records to determine if any major mistakes were made.
