Jan Adams Mug Shot, Controversy

More and more controversy is swirling around Jan Adams, the doctor who performed the surgery on Donda West that quickly led to her death.

Here's the mug shot of the bad doc, courtesy of his 2006 arrest for DUI.

Moreover, TMZ has learned that Dr. Adams is not even Board Certified with the American Board of Plastic Surgery.

There have also been two malpractice judgments filed against Dr. Adams, both in 2001. One was for $217,337 and the other for $250,000.

This guy is shadier than Cisco Adler.
